To place an order by phone, please call our Customer Service at one of the above numbers; one of our representatives will be happy to take your order.
Please have on hand all the information required to open or identify your account including your password. It is important to know the quantities and numbers of the products you wish to order. You will also need to provide your credit card number so we can process your pre-authorized payment.
You will be sent an order confirmation by fax or email.

To place an order by fax, please fax us your completed order form at the above number. Our Customer Service can send you a copy of the order form or you can download it from here.
